Apex-Healthcare in West Palm Beach, FL seeks an experienced RN MDS Coordinator to join our Skilled Nursing Facility team. You will manage MDS assessments, ensure RAI timelines, and coordinate with interdisciplinary staff to support resident care and regulatory compliance.

The role emphasizes accuracy, teamwork, and education of staff on documentation standards, CMS guidelines, and reimbursement requirements. Florida RN license and MDS experience preferred; RAC-CT certification a plus.

Qualifications

• Active Florida RN license required.
• Previous MDS Coordinator experience in a Skilled Nursing Facility preferred.
• Strong knowledge of RAI, Medicare, and CMS regulations.
• Experience with care planning and interdisciplinary team collaboration.
• Excellent communication, organization, and documentation skills.
• RAC-CT certification is a plus.

Responsibilities

• Coordinate and complete MDS assessments in accordance with federal and state regulations.
• Ensure timely and accurate completion of the Resident Assessment Instrument (RAI) process.
• Collaborate with nursing, therapy, social services, and other departments to develop and update care plans.
• Monitor Medicare and reimbursement documentation requirements.
• Maintain compliance with CMS guidelines and facility policies.
• Participate in quality improvement initiatives and survey readiness activities.
• Educate staff on MDS processes and documentation standards.
