Job summary

Horizon Health and Wellness, Inc. provides healthcare services across central and southern Arizona, dedicated to improving lives with Kindness Matters.

We are looking for a Care Manager to coordinate services, advocate for clients, and ensure timely, effective care delivery in a fast-paced environment. The role requires at least a high school diploma and experience in behavioral health, with additional certifications preferred.

Qualifications

• HS diploma or GED plus 4 years in Behavioral Health or combined Behavioral Health education and experience to meet Behavioral Health Technician qualifications.
• One year case management experience strongly preferred.
• Ability to create plans and summaries based on client needs.
• Certification in first aid, CPR and Crisis De-escalation; training offered if not possessed upon hire.
• Must be at least 21 years of age or older.
• Possess valid driver's license with acceptable driving record and reliable transportation.
• Prior experience in the behavioral health setting is strongly preferred.
• Demonstrated organizational abilities to maintain communication, coordination, and connections with clinical team participants.
• Willingness to work evenings as needed.
• Desire and capacity to maintain time-management and organization while working in fast-paced environment.
• Must obtain and maintain a fingerprint clearance card and pass a pre-employment drug and background check.
• Have a Tuberculosis screening test and provide evidence of freedom from infectious TB prior to providing services; annual TB testing/screening may be performed.
• Possess the physical, mental, and cognitive ability to perform specific clinical tasks without posing a threat to safety.
