Job summary

Total Care Therapy in Hilliard, OH is seeking a Speech-Language Pathologist to evaluate, diagnose, and treat communication, cognitive-linguistic, and swallowing disorders while developing individualized treatment plans that improve residents\' quality of life and functional independence.

You will evaluate residents, create plans of care, document in the EMR, educate families and staff, and collaborate with an interdisciplinary team to deliver compassionate, evidence-based therapy to older adults

Qualifications

• Current State Speech-Language Pathologist (SLP) License.
• CCC-SLP preferred (or eligibility).
• Current CPR/BLS Certification.
• Valid driver\'s license, reliable transportation, and auto insurance.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized speech therapy plans of care.
• Assess and treat speech, language, voice, cognitive-communication, and swallowing disorders.
• Perform dysphagia evaluations and implement swallowing interventions.
• Reassess patient progress and adjust treatment plans as needed.
• Document timely and accurate notes in the EMR.
• Educate residents, caregivers, and staff on communication strategies and home exercise programs.
• Collaborate with physicians, nursing staff, dietitians, and the interdisciplinary team for coordinated care.
• Maintain a productive clinical caseload while delivering evidence-based therapy.
