Job summary

Continuum Therapy Partners, LLC seeks a licensed Speech-Language Pathologist to provide patient-centered assessment and treatment at Willowdale Nursing & Rehab Center in New Holstein, WI. The role requires CCC-SLP and an active state license.

This full-time daytime position offers a comprehensive benefits package, licensure reimbursement, and a $10,000 retention bonus to support long-term commitment. You will evaluate speech, language, and swallowing disorders, develop personalized treatment

Qualifications

• Master's degree in Speech-Language Pathology is required.
• Current, unencumbered state SLP license; ASHA CCC-SLP required.
• 1 year of experience as an SLP preferred.

Responsibilities

• Evaluates and develops individualized treatment plans for speech, language, and swallowing disorders.
• Documents patient progress and adjusts treatment plans based on outcomes.
• Educates patients and families on communication strategies and swallowing safety.

Speech-Language Pathologist Requirements & Responsibilities

• Master's Degree in Speech-Language Pathology required
• Current, unencumbered state SLP License; ASHA Certificate of Clinical Competence (CCC-SLP) required.
• 1 year of previous experience as a Speech-Language Pathology preferred
• Evaluates and develops individualized treatment plans for speech, language, and swallowing disorders
• Documents patient progress and adjusts treatment plans based on clinical outcomes
• Educates patients and families on communication strategies and swallowing safety
