Job description

BAYADA Home Health Care is currently seeking a Speech Language Pathologist (SLP) for a per diem opportunity performing home health visits. This office services adult and geriatric patients in homes throughout Greenville, Spartanburg, and Anderson counties.

As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to receive the necessary medical care to remain at home.

Responsibilities of the Speech Language Pathologist, SLP:

• Make home visits to clients in designated geographic areas.
• Perform evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
• Develop treatment plans based on evaluations, physician’s orders, and goals.
• Assess and revise the speech therapy care plan regularly, and collaborate with BAYADA clinicians in the multidisciplinary care plan as appropriate.
• Educate patients, family members, or caregivers on rehabilitative care and activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using a state-of-the-art touchpad tablet.

Qualifications for the Speech Language Pathologist, SLP:

• Current license as a Speech Language Pathologist in South Carolina.
• Certificate of Clinical Competence in speech from the American Speech-Language-Hearing Association, or
• Equivalent education and work experience qualifying for such certification, and awaiting certification exam.
• Completed education and work experience requirements, with supervised experience in progress for certification.
• Strong computer skills; prior experience with electronic medical records (EMR) preferred.
