BAYADA Home Health Care is currently seeking a Speech Language Pathologist, SLP, for a per diem opportunity performing home health visits. This office services adult and geriatric patients in homes throughout Cleveland County.

As a home care Speech Language Pathologist, SLP, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Responsibilities:

1. Make home visits to clients in designated geographic territories.
2. Perform evaluations to assess, diagnose, and treat speech, language, communication, or swallowing difficulties.
3. Develop and revise treatment plans based on evaluations, physician’s orders, and goals.
4. Participate in multidisciplinary care planning with BAYADA clinicians.
5. Educate patients and family members on rehabilitative activities to promote health, safety, and independence.
6. Document observations, interventions, and evaluations accurately using electronic medical records.

Qualifications:

1. Current license as a Speech Language Pathologist in NC.
2. Certificate of clinical competence from the American Speech and Hearing Association or equivalent education and work experience, with eligibility to take the ASHA exam.
3. Solid computer skills; prior EMR experience preferred.
