BAYADA Home Health Care - Speech Language Pathologist, SLP (Per Diem)

BAYADA Home Health Care is seeking a Speech Language Pathologist (SLP) for a per diem position providing home health visits in Cleveland County, serving adult and geriatric patients.

As a vital part of a multidisciplinary team, you will deliver rehabilitative care and skilled nursing to enable clients to remain at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform evaluations to assess, diagnose, and treat speech, language, communication, or swallowing issues.
3. Develop and update treatment plans based on evaluations, physician's orders, and goals.
4. Collaborate with clinicians in multidisciplinary care planning.
5. Educate patients and families on rehabilitative activities to promote health, safety, and independence.
6. Document care and interventions accurately using electronic devices.

Qualifications:

1. Current NC license as a Speech Language Pathologist.
2. Certificate of Clinical Competence from ASHA, or equivalent education and experience, with eligibility to sit for the certification exam.
3. Strong computer skills; experience with EMR preferred.
