Job description

BAYADA Home Health Care is currently seeking a Speech Language Pathologist (SLP) for a per diem home health opportunity. This position involves visiting adult and geriatric patients in their homes throughout Lincoln County.

As a home care Speech Language Pathologist, you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing to help clients remain at home.

Responsibilities:

1. Make home visits to clients in designated areas.
2. Perform evaluations to assess, diagnose, and treat speech, language, communication, or swallowing issues.
3. Develop and revise treatment plans based on evaluations, physician’s orders, and goals.
4. Participate in multidisciplinary care planning.
5. Educate patients and families on rehabilitative activities to promote health and safety.
6. Document care activities accurately using a digital tablet.

Qualifications:

• Current NC Speech Language Pathologist license.
• Certificate of Clinical Competence from ASHA or equivalent educational and work experience, with eligibility to take the ASHA exam.
• Strong computer skills; experience with EMR preferred.
