Job summary

Western Slope Health Center is seeking a compassionate Speech-Language Pathologist to join our rehabilitation team in Placerville, CA. You will evaluate residents, develop individualized plans, and provide services to improve communication, cognition, and swallowing.

The role requires a CA SLP license, experience in geriatric care, and collaboration with an interdisciplinary team. Full-time position with a strong emphasis on resident-centered care and compliant documentation.

Qualifications

• Bachelor's degree in Speech-Language Pathology required; graduates with Master's or Doctoral in Speech-Language Pathology preferred.
• Experience in skilled nursing, post-acute care, rehabilitation, or geriatric healthcare is preferred.
• Strong understanding of speech, language, cognitive, communication, and swallowing disorders.
• Experience with an interdisciplinary healthcare team preferred.
• Proficiency with electronic medical record systems; Net Health and PointClickCare preferred.

Responsibilities

• Evaluate residents to determine therapy needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Provide SLP services to improve communication, cognition, swallowing, safety, and independence.
• Assist nursing staff with training and education of Restorative Aides.
• Supervise CFY and/or students as applicable per state requirements.
• Communicate progress and discharge plans with physicians and families.
• Maintain accurate medical records and compliant documentation.
• Follow infection control and safety protocols.
