Job summary

The Pines at Placerville Healthcare Center in Placerville, CA is seeking a per diem Speech Language Pathologist to join our compassionate rehabilitation team. You will assess residents to identify speech, language, cognitive, and swallowing needs and develop individualized therapy plans to improve communication and safety.

Responsibilities

Evaluate residents to determine speech, language, cognitive, and swallowing therapy needs.
Develop and implement individualized treatment plans.
Work closely with physicians, nursing staff, families, and interdisciplinary teams.
Educate residents and caregivers on aspiration prevention, communication strategies, and maintenance programs.
Participate in care conferences, rehabilitation meetings, and utilization reviews.
Maintain accurate and compliant documentation.
Supervise CFYs and students as applicable; assist with staff education and restorative nursing training.
Help residents maximize independence, safety, and quality of life.
