Job summary

Woburn Rehab & Nursing Center seeks a full-time Speech-Language Pathologist (SLP) to join our rehabilitation team. The SLP will evaluate residents, develop individualized treatment plans, and provide direct therapy to improve communication and swallowing function in a resident-centered setting.

The role requires state licensure, a Master’s in Speech-Language Pathology, and a positive, collaborative approach within an interdisciplinary team.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current state licensure as a Speech-Language Pathologist.
• CCC-SLP certification preferred.

Responsibilities

• Evaluate residents referred for speech-language pathology services.
• Assess speech, language, communication, cognitive, and swallowing disorders.
• Develop individualized treatment plans based on each resident's needs, abilities, and rehabilitation goals.
• Provide direct therapy to improve communication, cognition, and swallowing function.
• Establish measurable and clinically appropriate treatment goals.
• Monitor resident progress and modify treatment plans as needed.
• Complete evaluations, progress notes, recertifications, and discharge documentation accurately and on time.
• Educate residents, families, and caregivers on treatment plans and therapeutic techniques.
• Collaborate with physicians, nurses, physical therapists, occupational therapists, and other members of the interdisciplinary team.
• Maintain accurate and complete clinical documentation.
• Follow all applicable federal, state, and facility regulations, including HIPAA and resident rights requirements.
• Perform other duties as assigned by the Director of Rehabilitation.
