Job summary

Continuum Rehab Group seeks a Speech Language Pathologist CF or CCC to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in Ft. Worth, TX. The role emphasizes evaluation, treatment planning, and collaboration with a multidisciplinary team to improve patients’ communication, cognition, and swallowing safety.

Qualifications

• Responsible for evaluation and treatment of patients with deficits within the covered scope of an SLP.
• Focus on increasing intelligibility of speech and using augmentative devices to facilitate communication.
• Addressing cognitive functions and receptive-expressive language.
• Improve swallowing safety through oral exercises and texture modification.

Responsibilities

• Direct all aspects of quality patient care including evaluation, treatment planning, goal setting, family education, and documentation.
• Provide professional treatment for speech, language, hearing, oral motor, swallowing, and cognitive disorders.
• Plan activities and provide services for speech and language disorders.
• Perform evaluations and develop treatment plans with short- and long-term goals.
• Collaborate with occupational and physical therapists, physicians, and other healthcare team members.
• Document evaluations, treatment progress notes, and discharge summaries promptly.
• Ensure delivery of quality rehab services in the SNF.
• Maintain compliance with company policies and code of conduct.
