Job summary

Continuum Rehab Group in Waco/McGregor, Texas is seeking a Speech Language Pathologist CF or CCC to join our in-house therapy team serving Skilled Nursing and Long-Term Care facilities.

You will evaluate and treat patients, focusing on intelligibility, swallowing safety, and communication needs, while collaborating with the rehab team and documenting progress.

Qualifications

• Experience evaluating and treating patients with speech and language disorders.
• Ability to use alternative communication strategies and manage swallowing safety.
• Experience in SNF/LTC settings preferred.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, planning, goal setting, family education, and documentation.
• Provide professional treatment for rehabilitation of speech, language, hearing, oral motor, swallowing, and cognitive disorders.
• Plan activities and provide therapeutic services for speech and language disorders.
• Develop treatment plans with goals aligned to physician orders and patient condition.
• Document evaluations, progress notes, and discharge summaries promptly.
