Job summary

Continuum Rehab Group seeks a Speech Language Pathologist CF or CCC to join our in-house therapy team serving Skilled Nursing/Long Term Care Facilities in St. Louis, MO.

You’ll evaluate and treat residents to improve speech intelligibility and swallowing safety, while addressing cognitive and language needs and coordinating with the care team. Qualifications

• Evaluations and treatments for speech-language disorders as part of SNF care.
• Ability to work with a multidisciplinary team in long-term care settings.
• Licensure requirements for SLP in the state of Missouri.

Responsibilities

• Direct patient evaluations, develop treatment plans and goals.
• Provide speech, language, cognitive, and swallowing therapy as prescribed.
• Document progress notes and discharge summaries per policy.
• Collaborate with therapists and physicians on patient plans of care.
