Qualifications

• Successful candidates will be eligible for CF/CCC supervision and licensure.
• Excellent interpersonal and written communication skills are essential.

Responsibilities

• Direct evaluation and treatment of patients with speech, language, cognitive, and swallowing needs.
• Develop and monitor Plan of Care and treatment goals.
• Collaborate with occupational and physical therapy and other care team members.
• Document progress, discharge plans, and outcomes in patient records.
• Ensure delivery of high-quality rehab services in the SNF setting.
