Job summary

Continuum Rehab Group in Rockwall, TX is seeking a Speech Language Pathologist CF or CCC to join our in-house therapy team serving Skilled Nursing and Long Term Care facilities. • Must hold SLP CF or CCC certification.
• Experience in skilled nursing or long-term care settings preferred.
• Strong evaluation and treatment planning skills.

Responsibilities

• Direct evaluation and treatment of patients with speech, language, swallowing, and cognitive disorders.
• Develop and monitor treatment plans and goals, per physician orders.
• Document progress notes, plans of care, and discharge summaries.
• Collaborate with OTs, PTs, physicians and care teams on patient programs.
• Maintain compliance with company policies and care standards.
