Rehabilitation Center of Oakland is seeking a dedicated social services professional to support discharge planning and care coordination in a skilled nursing facility. You will perform MDS-related tasks, ensure thorough documentation, and participate in daily meetings with the care team.

Qualifications

• Must have experience in a skilled nursing facility.
• MDS completion.
• Excellent documentation.

Responsibilities

• Coordinate discharge planning.
• Complete MDS nursing assessments and plan.
• Attend IDT and care plan meetings.
• Interact with residents, families, physicians, hospice and home health representatives.
• Collaborate with management team.
• Coordinate room changes as needed.
