Job summary

Hc is seeking a compassionate Social Worker/Discharge Planner to support residents and families through admission to discharge in a Skilled Nursing Facility.

The role requires leadership in discharges, timely data gathering, and collaboration in multidisciplinary teams. Proficiency with EMR systems (PCC, MDS 3.0 a plus) and strong communication will drive resident e xperience and quality of life.

Qualifications

• Bachelor's degree in Social Work or related field; MSW preferred.
• Experience in Social Service office is a plus but not required.
• Familiarity with discharge planning and community resources.
• Knowledge of discharge planning and resources; strong communication.

Responsibilities

• Attend daily morning meetings and weekly UR meetings.
• Gather discharge information within 48 hours of admission.
• Plan and attend discharge care plan meetings; address discharge concerns.
• Document discharge notes in ECS; complete discharge audit forms.
• Assist with annual post form audit and discharge-related tasks.
• Address psychosocial needs and coordinate care plans; facilitate safe transitions.

Social Worker/Discharge Planner

A well-established Skilled Nursing Facility is seeking a compassionate and driven Social Worker/Discharge Planner to join our team. This role is ideal for someone who is passionate about supporting residents and their families while ensuring a smooth care experience from admission through discharge.

The position requires attending daily morning meetings, weekly UR meetings, leading, planning and completing all discharges. Gathering discharge information from new skilled patients within 48 hours of admission. Planning and attending discharge care plan meetings, addressing discharge concerns, documenting discharge notes in ECS, completing discharge audit forms, assist with annual post form audit. Social Worker is responsible for addressing the psychosocial needs of residents, coordinating care plans, and facilitating safe and effective transitions. This position plays a key role in enhancing resident quality of life and overall satisfaction.

Qualifications/Requirements

• Bachelor's degree in Social Work or related field (Psychology, Nursing or Care Management); MSW preferred


• Experience in Social Service office preferred, but not required


• Prior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred


• Strong communication and interpersonal skills


• Knowledge of discharge planning and community resources


• Ability to manage multiple responsibilities in a fast-paced environment


• Familiarity with EMR systems; PCC and MDS 3.0 experience a plus

Salary Range: $65,000-$80,000 a year (based on Experience)
