Qualifications

• Bachelor’s degree is required.
• Experience as Social Services Director in a skilled nursing facility or long-term care setting is required.
• Strong knowledge of discharge planning, psychosocial assessments, care planning, resident rights, and SNF operations.
• Strong written and verbal communication skills.
• Excellent organization, documentation, and follow-through.

Responsibilities

• Assess residents’ psychosocial needs and develop interventions and care plans.
• Participate in interdisciplinary care plan meetings and coordinate with Nursing, Rehabilitation, Activities, Dietary, and more.
• Coordinate discharge planning and assist with community resources, placement, transportation, equipment, and post-discharge needs.
• Support residents and families with adjustment to placement and psychosocial concerns.
• Assist with resident grievances and ensure timely resolution.
• Maintain accurate social services documentation per policy and regulations.
• Support residents’ rights, dignity, and quality of life.
• Communicate effectively with residents, families, physicians, hospitals, and external providers.
• Ensure the Social Services Department remains compliant with federal and California SNF requirements.

Ocean Park Healthcare Center and Sunset Park Healthcare Center are seeking an experienced Social Services Director to join our team. We are looking for a compassionate, organized, and experienced professional who understands the unique needs of residents in a skilled nursing environment and can effectively coordinate psychosocial services, discharge planning, and resident advocacy.

Responsibilities

The Social Services Director is responsible for overseeing and coordinating social services for our residents, including:

• Assessing residents’ psychosocial needs and developing appropriate interventions and care plans.

• Participating in interdisciplinary care plan meetings and collaborating closely with Nursing, Rehabilitation, Activities, Dietary, and other departments.

• Coordinating discharge planning and assisting residents and families with community resources, placement, transportation, equipment, and other post-discharge needs.

• Supporting residents and families with adjustment to placement, changes in condition, and other psychosocial concerns.

• Assisting with resident grievances and ensuring concerns are addressed promptly and appropriately.

• Maintaining accurate and timely social services documentation in accordance with facility policies and regulatory requirements.

• Supporting residents’ rights, dignity, choice, and quality of life.

• Communicating effectively with residents, families, physicians, hospitals, community agencies, and members of the interdisciplinary team.

• Ensuring the Social Services Department remains compliant with applicable federal and California skilled nursing facility requirements.

Required Qualifications

• Bachelor’s degree required.

• Previous experience as a Social Services Director in a skilled nursing facility or long-term care setting required.

• Strong knowledge of discharge planning, psychosocial assessments, care planning, resident rights, and skilled nursing facility operations.

• Strong written and verbal communication skills.

• Excellent organization, documentation, and follow-through.

• Ability to work collaboratively with residents, families, staff, and outside providers.

• Ability to manage multiple priorities in a fast-paced healthcare environment.

We are looking for someone who is dependable, compassionate, proactive, and comfortable taking ownership of the Social Services Department. The ideal candidate will have strong SNF experience and understand the importance of both resident-centered care and regulatory compliance.
