Job summary

Evolve Therapy Services in Le Center, MN is seeking a compassionate Physical Therapist to join our in-house Skilled Nursing Facility partner. This position is offered as Part Time or PRN and requires delivering PT services to residents in accordance with standards and CMS expectations.

You will evaluate residents, develop treatment plans with short- and long-term goals, document thoroughly, and collaborate with an interdisciplinary team.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Interpret evaluation findings and communicate recommendations to residents and team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
