Job summary

Evolve-Therapy-Services is seeking a Physical Therapist for our in-house Skilled Nursing Facility. You will deliver PT services, screen residents, and develop targeted treatment plans while adhering to CMS guidelines and company procedures.

This role requires a current PT license and a BS in Physical Therapy. You will collaborate with physicians, nurses, and therapy staff, documenting progress and adjusting plans as needed to ensure high-quality resident care.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Commitment to ongoing continuing education and professional development to maintain licensure and certification.
• Experience in skilled nursing facility settings is a plus

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for PT services.
• Develop treatment plans with short and long-term goals and track progress.
• Communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Screen residents regularly to determine need for intervention/treatment.
