PATH (People Assisting The Homeless) is seeking a Case Manager at the Palomar Point PSH site in Chula Vista, CA. The role focuses on helping clients break the cycle of homelessness through coordinated services, housing placement, and access to healthcare and social supports.

The position requires compassionate service, strong data-driven decision making, and the ability to navigate CES and housing systems. Driving is an essential function; full-time, non-exempt status with health and wellness
