Job summary

Chatham Hills Subacute Care Center in Madison, NJ is seeking an Occupational Therapist to join our growing rehabilitation team. You will assess referred patients, develop and supervise treatment plans, and work under the physician to help patients regain function.

Candidates must be licensed OT with experience in long-term or geriatric care, strong documentation, and teamwork. This full-time role offers a $5,000 sign-on bonus and a patient-centered environment.

Qualifications

• Graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Responsibilities

• Assess referred patients requiring rehabilitation services and provide direct patient care to assess medical condition and functional capabilities.
• Establish and administer a treatment program with goals determined by patient capacity and tolerance under physician direction.
• Set realistic goals, document patient condition and educate patients and families in appropriate therapy methods.
