Job summary

The Chatham Hills Subacute Care Center in Cranford, NJ is seeking an Occupational Therapist to join our rehabilitation team. You will assess referred patients, provide direct care, and develop treatment programs with goals aligned to patient capacity under physician direction.

Must hold an active OT license; experience in long-term or geriatric care is preferred. A $5,000 sign-on bonus is offered along with opportunities for interdisciplinary collaboration and patient-centered care.

Qualifications

• Graduate of an accredited Occupational Therapist program.
• Active state license and registration for state of practice.
• Oversee and supervise OT Assistants.
• Experience in long-term care or geriatric care preferred.
• Strong documentation skills.
• Experience with evaluations, weekly progress notes, re-certification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Demonstrates professional conduct and clinical excellence.
• Punctual, reliable and adheres to schedules.

Responsibilities

• Assess referred patients requiring rehabilitation services and provide direct patient care.
• Develop and administer treatment programs with goals based on patient capacity and tolerance under physician direction.
• Set realistic goals, document patient condition and educate patients and families.
• Record progress and communicate with the interdisciplinary team.
• Assist and supervise Occupational Therapy Assistants as needed.
