Job summary

Chatham Hills Subacute Care Center is seeking a skilled Occupational Therapist to join our rehabilitation team in New Jersey. The OT will assess referred patients, develop and administer treatment plans, and provide direct patient care with measurable goals under physician supervision.

The role requires state OT licensure, comfort supervising OT Assistants, and a focus on geriatric and long-term care patients. Strong documentation and teamwork are essential for high-quality patient-centered care.

Qualifications

• Graduate of an accredited Occupational Therapist program.
• Must hold valid OT license and state registration.
• Experience supervising OTAs is required.
• Experience in long-term/geriatric care preferred.
• Strong documentation writing skills.
• Proficient in patient evaluations, progress notes, and discharge documentation.

Responsibilities

• Assess referred patients and develop rehabilitation plans.
• Provide direct patient care with goals set under physician direction.
• Document patient condition and educate patients and families.
• Supervise and guide Occupational Therapy Assistants.
• Maintain confidentiality and comply with HIPAA and Residents' Rights.
