Chatham Hills Subacute Care Center in New Jersey is seeking an Occupational Therapist to join our rehabilitation team. You will assess referred patients, develop personalized treatment plans, and work under physician direction to help improve functional abilities.

You will document patient status, educate patients and families on therapy methods, supervise OT Assistants, and collaborate with other disciplines to provide patient-centered care in a long-term/geriatric setting.

Formación

• Graduated from an accredited OT college or university.
• Must hold a valid OT license and state registration.
• Oversee OT Assistants in the unit.
• Experience with long-term/geriatric care preferred.
• Strong documentation writing skills.
• Experience with evaluations, progress notes, re-certification and discharge documents.

Responsabilidades

• Assess referred patients to determine rehabilitation needs.
• Develop and administer individualized treatment programs.
• Set realistic goals based on patient capacity and tolerance.
• Document patient condition and progress.
• Educate patients and families on therapy methods.
