Chatham Hills Subacute Care Center is seeking an Occupational Therapist to join our growing rehabilitation team in New Providence, NJ. You will assess patients requiring rehabilitation, design individualized treatment plans, and implement goal-oriented therapy under physician supervision.

The role emphasizes documentation, collaboration with the care team, and patient/family education. Long-term or geriatric care experience is preferred, and a valid OT license is required.

Formación

• Graduated from an accredited Occupational Therapist program.
• Valid professional license and state registration.
• Oversee and supervise Occupational Therapy Assistants.
• Experience with long-term or geriatric care is preferred.
• Strong documentation writing skills.
• Ability to collaborate across disciplines and document patient conditions.

Responsabilidades

• Assess referred patients needing rehabilitation.
• Develop and administer individualized treatment plans.
• Set goals based on patient capacity and tolerance under physician direction.
• Document patient status and progress; educate patients and families.
