Job summary

Holy Redeemer is seeking an admissions coordinator to oversee all aspects of care for short-term rehab patients from admission through discharge. You will ensure patients' strengths are leveraged and that they understand available options during their stay, while delivering exceptional customer service.

Applicants should hold a Bachelor's degree in Social Work or related field, with strong communication and computer skills; prior admissions/discharge experience in long-term care is preferred.

SUMMARY OF JOB

Coordinates all aspects of care and services for short term rehab patients beginning with admission process to discharge, ensuring each patient's' strengths and abilities are maximized. Provides excellent customer service in which patients are educated and informed of all of their options during their stay.

RECRUITMENT REQUIREMENTS

Bachelor's degree in Social Work, Human Services or Human Services related field

Excellent communication skills and computer skills

Previous experience in the admissions/discharge for long term care preferred

Knowledge of community resources
