Redeemer Health in the United States is seeking a Social Services Coordinator to assess and coordinate the needs of residents, patients, and families, working with the interdisciplinary team to ensure appropriate care is provided.

You will support patients in improving and maintaining their physical, mental, and psychosocial well-being, collaborating with staff and families to promote person-centered care and independence.

Qualifications

• BSW or BA in sociology/social work or related human services/healthcare field.
• Minimum one year experience in a long-term care setting or equivalent related healthcare experience
• Excellent oral and written communication skills
• Knowledge of community resources
• Experience working as part of an interdisciplinary team
• Computer experience

Responsibilities

SUMMARY OF JOB

Social Services coordinator position identifies the needs of each resident/patient/families and assures that the needs are met by the appropriate discipline. Assists patients/residents/families in improving /maintaining and their ability to manage their physical, mental and psychosocial needs.
