Job summary

Redeemer Health seeks an RN to manage Hospice admissions across the community, nursing facilities, and inpatient units. You will coordinate the interdisciplinary plan of care with patients and families to ensure compassionate, patient-centered end-of-life care.

BSN preferred; CHPN preferred. Requires at least 1 year of med/surg experience and 1 year in home care or hospice. Flexibility with hours, including weekends, holidays, and night shifts, in a Southeastern PA/NJ service area.

Qualifications

• Graduate from an accredited school of nursing (RN licensure required).
• BSN preferred; CHPN preferred.
• Minimum 1 year med/surg experience; 1 year home care or hospice preferred.
• Ability to work flexible hours, including weekends, holidays, and night shifts.

Responsibilities

• Perform hospice admissions in the community, nursing homes, assisted living and inpatient hospice settings.
• Completes revisits to hospice patients as requested by clinical supervisor.
• Develops, implements, and coordinates the interdisciplinary plan of care for each patient and family.
• Participates in professional development activities related to end-of-life care.

SUMMARY OF JOB

Performs Hospice admissions in the community, nursing home, assisted living and inpatient hospice settings. Completes revisits to hospice patients as requested by clinical supervisor. Develops, implements, and coordinates the interdisciplinary plan of care for each patient and family. Participates in professional development activities that further personal and program goals related to end of life care.

RECRUITMENT REQUIREMENTS

Graduate from an accredited school of nursing.

BSN preferred

CHPN preferred

Minimum 1 year med/surg experience.

Minimum 1 year home care or hospice experience preferred.

Ability to work flexible hours, and meet weekend, holiday, and NOC requirements

Current IV and venipuncture experience preferred.

LICENSE AND REGULATORY REQUIREMENTS:

Current licensure in State of practice, Valid Driver’s License, Current CPR Certification
