Job summary

Harbour House Family Center - New Bedford, MA is seeking a Case Manager to provide direct care to residents, conduct assessments, and develop treatment and housing plans. You will manage a caseload, perform counseling, coordinate referrals, and ensure compliance with regulations and documentation standards.

Requirements include a high school diploma or GED (associate's preferred), valid driver's license, willingness to travel, and strong communication skills.

Qualifications

• Must present as courteous and professional at all times.
• Excellent communication skills and ability to organize ideas clearly.
• Knowledge of basic computer skills and EHR systems.
• Ability to work with others in cooperative and collaborative manner.
• Understanding of the relationship between substance use and mental health issues.
• Maintain therapeutic boundaries and adhere to rules respectfully.
• Comply with Federal Regulations (42 CFR Part 2) and HIPAA.
• TB/Flu and Covid-19 assessments and de-escalation training completed.
• Successful completion of trainings upon hire and annually.

Responsibilities

• Maintain assigned caseload and provide direct resident care.
• Conduct assessments, counseling, and develop treatment/discharge plans.
• Make referrals and coordinate aftercare services.
• Keep medical records complete, timely, and accurate.
• Link residents with housing and aftercare services post-discharge.
• Build community partnerships to support sobriety and housing stability.
• Attend treatment team meetings and mandatory trainings.
