Job summary

High Point & Affiliated Organizations seeks a dedicated Case Manager to support transitional housing residents in New Bedford. You will assist with intakes, ISP development, referrals, and documentation while maintaining confidential case files.

This role emphasizes strong communication, organization, and collaboration with local agencies. A High School Diploma or GED is required, with a driver’s license and experience in substance use treatment preferred. Training provided.

Qualifications

• High School Diploma or GED required; one year of substance use treatment experience preferred.
• Valid driver’s license and reliable transportation.
• Excellent communication and organizational skills; ability to document accurately.

Responsibilities

• Facilitate resident intakes and complete ISP with each resident.
• Maintain accurate case files and update ISP monthly.
• Meet bi-weekly with residents to review progress and update goals.
• Coordinate referrals to agencies and ensure compliance with regulations.
• Attend monthly case reviews and required trainings.
