Job summary

Pillars is seeking a full-time Shelter Case Manager to serve clients at the Adult & Family Shelter in Appleton, WI. You will perform case management to identify needs, create individual plans, and monitor progress toward housing and income stability.

The role emphasizes trauma-informed care, relationship-building, and collaboration with schools and community partners. Strong organization and advocacy are essential in this position.

Qualifications

• Bachelor's in Human Services related field or HS diploma with 5+ years relevant experience
• Minimum 2 years relevant experience preferred
• Experience with homelessness, domestic violence, mental health, or addictions insights
• Knowledge of community resources for low-income families experiencing homelessness
• Possess and maintain a valid Wisconsin driver’s license

Responsibilities

• Provide case management to individuals and families experiencing homelessness in a shelter setting
• Establish and monitor individual case plans to meet housing and income stability goals
• Advocate for participants across systems when needed
• Refer clients to community resources and act as a broker between resources
• Manage paper and electronic records and grant documentation
• Coordinate school enrollment and connect families to services like Head Start and McKinney-Vento provisions
• Represent the agency in community and interagency activities
