Job summary

Pillars Inc. is hiring a Street Outreach Case Manager (40 hours/week) in Appleton, WI. You will identify unsheltered individuals, perform face-to-face assessments, and develop case plans to help clients progress toward permanent housing.

Applicants should have a Bachelor’s in Human Services or a HS diploma with 5+ years of relevant experience. Local travel and a valid WI driver’s license are required. Pillars offers a robust benefits package and an AA/EOE stance.

Qualifications

• Bachelor's in Human Services related field or HS diploma with 5+ years experience
• Ability to travel locally and valid Wisconsin driver’s license with clean driving record

Responsibilities

• Conducts outreach to identify unsheltered individuals and assess needs
• Develops individual case plans and connects clients to housing resources
• Advocates for clients across systems and coordinates services
• Maintains records and reporting for grants and Pillars programs
• Represents Pillars in community outreach and interagency activities
• Maintains confidentiality and handles sensitive information
• Works collaboratively with a diverse team to reduce barriers to housing
