Job summary

Volunteers of America Upstate NY Ithaca Shelter is seeking a dedicated Case Manager to support residents of the single adult emergency shelter in developing individualized goals and securing permanent housing. This full-time, seasonal role emphasizes trauma-informed, person-centered care and requires coordination with housing providers and community partners.

Qualifications

• Bachelor's degree in human services or social work or equivalent required.
• Minimum 2 years of case management experience with homeless individuals.
• Knowledge of DHS and HUD guidelines preferred.
• CPR/First Aid certification must be obtained within 90 days of hire.

Responsibilities

• Complete intake assessments and develop individualized service plans within 48 hours of shelter entry.
• Review eligibility and orientations to ensure appropriate shelter placement.
• Provide crisis intervention and ongoing support to stabilize residents.
• Create transition plans to obtain permanent housing.
• Refer residents to medical, mental health, substance use treatment and other services.
• Assist with IDs, benefits, and housing documentation.
• Maintain a safe, trauma-informed shelter environment and document progress.
• Conduct regular one-on-one meetings to monitor progress and adjust plans.
• Lead life skills workshops, meetings, and group activities.
• Provide job readiness support and employment search assistance when appropriate.
• Transport residents to appointments and build landlord relationships.
• Maintain HMIS data accurately and timely.
• Report progress to Shelter Manager and participate in case conferences.
