Volunteers of America, Utah seeks a Shelter Case Manager to provide housing-focused, trauma-informed case management for youth at the Youth Resource Center. You will handle shelter intakes, support housing goals, and connect youth with internal and community resources.

Responsibilities include managing a caseload, coordinating housing options, and documenting outcomes in HMIS. The role requires strong boundaries, confidentiality, and regular on-site presence.

Qualifications

• Familiarity with housing-focused, trauma-informed, youth-centered practices.
• Willingness to qualify and pass Case Management Certification within 60 days of hire.
• Ability to uphold professional boundaries and confidentiality requirements.
• Ability to move throughout the facility and interact with clients for extended periods.
• Must be at least 21 years of age, possess a valid driver’s license and good driving record.
• Ability to lift and carry 50 lbs.

Responsabilités

• Complete shelter intakes, diversion/problem-solving conversations, and required intake assessments for youth.
• Maintain an assigned caseload and provide coverage for shelter services.
• Meet regularly with clients and maintain accessible case management within the shelter.
• Support housing navigation and placement activities, including applications and coordinating appointments.
• Provide referrals to internal and community resources (housing, benefits, employment, education, etc.).
• Document intakes, case management contacts, and outcomes in HMIS and other systems.
• Attend supervision, staff meetings, and trainings.
• Maintain professional boundaries and client confidentiality.
