3Rivers in Island Falls, ME is seeking a Shared Living Coordinator to oversee and support the Shared Living Program. You will collaborate with providers, individuals served, families, and community partners to ensure quality, person-centered services.

The role emphasizes planning, documentation, and compliance with Medicaid/HCBS requirements, with a focus on team collaboration and ongoing program development in the North Bangor region.

Formación

• Bachelor’s degree in Social Services or related field preferred.
• Equivalent experience may be substituted for education.
• Experience working with individuals with intellectual and developmental disabilities preferred.
• Excellent verbal and written communication skills.
• Ability to work independently with flexibility and manage multiple priorities.
• Maintain confidentiality per regulations and agency requirements.
• Proficiency in Microsoft Office programs (Word, Excel).
• Ability to motivate others and work as part of a team.
• Valid Maine driver’s license required.
• Ability to pass required background checks.

Responsabilidades

• Develop and maintain positive relationships with individuals supported and Contracted Home Providers to allow for observation, assessment, and support planning.
• Ensure all services are aligned with the individual’s PCP.
• Complete required home visits and contacts with Contracted Home Providers and individuals served.
• Maintain assigned caseload responsibilities as determined by Shared Living leadership.
• Provide individualized training and support to Contracted Home Providers as needed.
• Establish and maintain professional working relationships with individuals, families, staff, providers, and community partners.
• Ensure Contracted Home Providers deliver quality services per regulations and program requirements.
• Maintain accurate records to support compliance and best practices.
• Follow all reporting requirements and agency procedures regarding incidents and safety concerns.
