Job summary

Assistance Plus in Waterville, ME, is seeking a detail‑oriented Shared Living Coordinator to oversee our Shared Living Providers and ensure adult members receive high‑quality in‑home and community services. The SLC will work with case managers, providers, members and guardians, conduct home visits, maintain documentation, and ensure compliance with HCBS regulations and MaineCare standards.

• High school diploma or equivalent required; Associate's degree preferred.
• Must be 18 years of age or older.
• Proof of vehicle insurance at $50K/$100K and a valid Maine driver's license are required.

Responsibilities

• Recruiting and retaining Shared Living Providers.
• Conducting home visits to assess provider suitability.
• Entering data into Evergreen for reportable events.
• Maintaining contact with the Case Manager and Guardian as needed.
• Ensuring client plans drive service activities.
