Vista Care is looking for a Shared Living Coordinator to connect IDD individuals with safe Host Homes in a hybrid field-based role. Denver/Aurora area with up to 75% local travel. This full-time position pays base compensation with comprehensive benefits.

Responsibilities include recruiting and vetting providers, assessing needs, coordinating placements, and ensuring home safety to HUD/HQS standards. You will support families during the critical first 90 days and collaborate with healthcare

Formación

• High School Diploma is required.
• 18+ years old with a valid driver’s license and a clean driving record.
• Must be able to pass extensive background checks.

Responsabilidades

• Source, interview, and train Shared Living and Respite providers.
• Assess needs, facilitate meet-and-greets, and coordinate residential placements.
• Ensure home safety to HUD/HQS standards and coordinate with community agencies.
• Coordinate rapid emergency placements with nursing and leadership teams.
• Oversee the first 90 days of a new placement.
