Job summary

ATTUF Group LLC seeks dedicated individuals or families in Northern Colorado to open their homes as Host Home Providers (HHP). You will offer a safe, person-centered home for individuals with IDD, delivering 24/7 care in a contracted role that enables meaningful daily life.

Responsibilities include ADL support, medication assistance, ISP goal execution, documentation, and coordination with care teams. Training is provided, and compensation is based on client needs with monthly payments.

Qualifications

• Must be at least 21 years old.
• Must reside in Loveland, Fort Collins, Greeley, Longmont, Windsor, or Wellington, CO.
• Must have valid Colorado driver’s license, reliable vehicle, and insurance.
• Must pass criminal background checks and fingerprinting.

Responsibilities

• Provide 24/7 supervision and support based on client needs and level of care.
• Assist with activities of daily living and mobility.
• Support individuals in achieving ISP goals and community involvement.
• Maintain daily service documentation and participate in ISP reviews and team meetings.
• Communicate with ATTUF Group staff, case managers, guardians, and service coordinators.
