Job summary

Sample Supports is seeking a Residential Coordinator to support adults with IDD in host homes, family settings, and supported apartments. You will handle care coordination, provider oversight, advocacy, and team collaboration to ensure safe, person-centered services.

The role includes health monitoring, documenting outcomes, and leading planning with interdisciplinary teams. Three levels exist based on experience, with opportunities for growth and leadership.

Qualifications

• Bachelor’s degree in human services, human development, social services, psychology, or a related field preferred.
• At least three years of residential, case-management, management-level, or IDD services experience.
• Demonstrated success managing complex or high-acuity cases.
• Experience mentoring or training employees.
• Advanced knowledge of IDD regulations, compliance requirements, and residential program operations.
• Ability to support the Residential Manager and serve as a resource for other coordinators.

Responsibilities

• Manage an assigned caseload of adults receiving residential services.
• Monitor health, safety, rights, preferences, and quality of life.
• Build professional relationships with individuals, providers, families, and teams.
• Advocate for person-centered, high-quality services.
• Coordinate with families, guardians, case managers, advocates, and interdisciplinary teams.
• Monitor medications, appointments, health assessments, protocols, and follow-ups.
• Develop and implement service, safety, treatment, behavior, and support plans with leadership.
• Conduct health, safety, and welfare visits; perform annual home visits and inspections.
• Supervise, train, and support residential contractors; ensure documentation accuracy.
• Provide education and resources to individuals, families, providers, and teams.
• Participate in meetings, supervision, court hearings, and reviews.
• Complete accurate documentation and records within 24 hours or deadlines.
• Audit records for accuracy, completeness, and compliance.
• Follow policies and applicable regulatory requirements.
• Use sound judgment during emergencies and escalations; provide on-call coverage as needed.
• Implement client-specific requirements and use appropriate intervention when necessary.
• Report mistreatment or concerns as a mandatory reporter.
