Qualifications

Responsibilities

• Evaluates the patient's functional status and PT needs; assists in developing the therapy plan of care.
• Delivers PT interventions to improve mobility and function per plan of care.
• Documents progress notes and reports outcomes per policy.
• Educates patient and family about the plan of care and progress toward goals.
• Coordinates with the health care team and physician.
