Job summary

UnitedHealth Group's Caretenders VNA, part of LHC Group, is seeking a Physical Therapist in Home Health to assess patient needs related to mobility and endurance, determine a treatment plan, perform interventions, and evaluate progress to improve overall well-being.

This role requires a current physical therapy license, CPR certification, a valid driver’s license, and the ability to work in diverse home settings.

Qualifications

• Active Physical Therapy license in state of practice.
• CPR certification.
• Valid driver's license and access to a dependable vehicle or reliable public transportation.
• Ability to work in-home with diverse patient populations.

Responsibilities

• Evaluates patient functional status and PT needs; assists in plan of care development.
• Provides PT services within scope of practice and plan of care.
• Observes and reports response to treatment and changes in condition.
• Treats patients to relieve pain and restore function.
• Instructs patient and family regarding plan of care, limitations, and progress.
• Visits patients per Plan of Treatment and documents progress notes.
• Completes PT plan of care within state guidelines and reports outcomes.
• Performs evaluations and documents anticipated frequency of care.
