Job summary

Nurse On Call Home Health, part of LHC Group within the Optum family, is seeking a Physical Therapist to evaluate and treat patients in home health. You will assess functional status, develop treatment plans, and contribute to patient well-being while coordinating with the care team.

This role requires active PT licensure, CPR, and a valid driver's license with reliable transportation. The position offers comprehensive benefits and opportunities to grow within UnitedHealth Group's nationwide

Qualifications

Current Physical Therapy licensure in state of practice.
Current CPR certification.
Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

Evaluates the patient's functional status and assists the physician in the therapy plan of care.
Provides services within the scope of practice as defined by state laws, in coordination with the health care team.
Observes and reports patient responses to treatment and any changes to the supervising nurse or physician.
Treats patients to relieve pain, restore function, and maintain maximum functioning.
Educates patient and family/caregivers regarding the plan of care and progress.
Visits patients per Plan of Treatment and documents progress notes per policy.
Completes evaluations and develops the PT plan of care within state guidelines.
Reports outcomes and anticipated frequency of care.
