Qualifications

• Active physical therapy license in the state of practice.
• Current CPR certification.

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Develops and implements the PT plan of care.
• Documents progress notes per policy.
• Collaborates with the health care team to coordinate care.
• Performs visits and reports patient progress.
