Job summary

Caretenders VNA, part of LHC Group, is seeking a Physical Therapist in Home Health to assess patient needs, develop treatment plans, implement interventions, and monitor progress. You will collaborate with physicians and nurses to optimize patient outcomes in home settings.

Required: current Physical Therapy licensure in the practice state and a valid driver’s license; CPR certification is preferred or can be completed within 90 days of hire.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient’s functional status and PT needs.
• Assists physician in development of the therapy plan of care.
• Provides services within the scope of practice and plan of care.
• Observes and reports patient response to treatment to nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family regarding plan of care, limitations, and progress.
• Completes evaluations and develops PT plan of care within state guidelines.
