Job summary

Caretenders, a part of LHC Group, is seeking a Physical Therapist for Home Health to assess patients, determine treatment plans, perform interventions, and evaluate progress toward functional goals.

You will collaborate with physicians and the health care team, educate patients and families, document visits, and ensure compliant, high-quality care across diverse home settings.

This role requires valid PT licensure and a driver's license.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to dependable vehicle or public transportation
• Current CPR certification or ability to obtain within 90 days of hire

Responsibilities

• Evaluates the patient's functional status and PT needs; assists in developing the therapy plan of care
• Provides services within state-licensed scope and in coordination with the health care team
• Observes and reports patient response to treatment and changes in condition
• Treats patients to relieve pain, restore function, and maintain maximum functioning
• Instructs patient and family regarding plan of care, limitations, and progress
• Completes progress notes for each visit and documents per policy
• Develops PT plan of care within state guidelines and reports outcomes
• Ensures care is delivered to diverse patients regardless of background
