Job summary

Caretenders, a part of LHC Group, a leading post-acute care partner, is seeking a Physical Therapist for Home Health. You will assess patient needs, develop a plan of care, and deliver interventions to improve mobility, endurance, and function in patients' homes.

Responsibilities include evaluating functional status, coordinating with the care team, documenting progress, and educating patients and families about the plan of care. Eligible candidates hold PT licensure in their state.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle or reliable public transportation.
• CPR certification or ability to obtain within 90 days of hire (preferred).

Responsibilities

• Evaluates the patient's functional status and needs; assists in care plan development.
• Delivers PT services within scope of practice and in coordination with the health care team.
• Observes and reports patient response to treatment to supervising staff.
• Treats to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient and family about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and documents progress notes.
• Completes evaluations and develops PT plan within state guidelines.
• Reports outcomes and anticipated frequency of care.
• Function in any home setting regardless of age, race, creed, color, sex, disability, or financial condition.
