Job summary

Moms In Motion / At Home Your Way is seeking a Service Facilitator Case Manager to visit clients in Virginia homes and support families navigating waiver programs. You’ll build plans of care, troubleshoot authorizations, and foster trusting relationships while working remotely from a laptop/tablet and traveling locally.

The role emphasizes compassion, organization, and community impact, with no degree required and a driver’s license as a must-have requirement.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; we care more about heart and experience.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license and a reliable vehicle.

You’ll spend your days connecting with families, helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and making sure those you serve can live safe, healthy, and independent lives.

What We’re Looking For

• A valid driver’s license & reliable vehicle (no client transport).
• Great communication—both written and spoken.
• Able to pass a background check + provide 2 professional references.
• Bonus points if you’ve got Person-Centered Thinking/Planning training.

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
