Job summary

Moms In Motion seeks a Service Facilitator to visit families in Virginia, helping them navigate Medicaid Waiver programs and ensuring safe, healthy, independent living. You’ll work locally visiting client homes and coordinating plans of care that make a real difference.

The role requires strong communication, two years of experience with disabilities or elderly care, and a valid driver’s license. No degree required and paid training provided.

What We’re Looking For

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required (we care more about heart and experience).
• Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• A valid driver’s license & reliable vehicle (no client transport).
• Great communication - both written and spoken.
• Able to pass a background check + provide 2 professional references.
• Bonus points if you’ve got Person-Centered Thinking/Planning training.

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
